At ECCG, we offer Individualized Home Supports (IHS) for clients who are currently enrolled in a waiver program, as well as Personal Lifestyle Supports for those who are not enrolled in a waiver but are looking for a private pay option. Individualized Home Supports with Training

Individualized Home Supports (IHS) with Training serve individuals who are living in their own home or in another independent living arrangement, and receive less than 24/7 supports.

Our IHS team works with each individual to
improve independence at home, socialization,
and safety skills within the community.

Individualized Home Supports with Family Training

Our Individualized Home Supports (IHS) with Family Training services include assistance to develop, maintain, and improve skills related to a client’s daily living. This includes working with the client on both independent living skills and family relations.

• Personal hygiene and cleanliness
• Setting up a routine to complete
• Household chores

Personal Lifestyle Supports - Private Pay

Personal Lifestyle Supports – Private Pay Services allow us to serve individuals who are not eligible for waivered services in Minnesota, but who are looking to enrich their lives with services within their own home and community. Our Private Pay services are designed to be flexible in order to meet each individual’s unique needs, including but not limited to:

• Community outings and integration
• Socialization and Companionship
• Assistance with navigating additional private in-home services
• Assistance with setting up and getting to healthcare appointments
• Light household duties

Frequently Asked Questions

How do I qualify for your services?

In order to qualify for our services, you must currently have an open CADI, BI, or DD waiver and have a referral sent to us from your Case Manager. If you’re not currently on a waiver but are interested in a more personalized approach, we’d be happy to share our private pay options with you

Can I call you for day-of services?

Typically we cannot offer day-of services, however we are usually able to accommodate requests the same week. In general, we have found that most successful client pairings happen when a client and staff member are able to meet weekly at a consistent time.

Can you cook/clean/shop for me?

No. Our services are designed to support you with these tasks—not do them for you. We work alongside you to help teach and build independence in these areas.   If you’re looking for more “for you” support, such as someone to handle   these tasks directly, we recommend asking your case manager to explore Homemaking or PCA services. 

You can also inquire about our private pay services for additional support options tailored to your needs!

Do you have any events for clients?

Yes! We host Connections events monthly. These are opportunities for you to participate in an activity alongside others who receive our services. Examples of past events have included guided painting classes, making flower arrangements, and creating vision boards.

Can I get services on the weekend?

Oftentimes, yes! However, we do typically need more time to find staff who have this availability.

Can my staff give me medication?

No. We would recommend asking your Case Manager for a referral for Skilled Nursing services.

Can my staff assist me with personal cares?

No. Our staff assist with IADL’s or Instrumental Activities of Daily Living—this means we help do tasks alongside our staff. If you are needing assistance with ADL’s or Activities of Daily Living (such as dressing, lifting, or showering), we encourage you to reach out to your Case Manager to make a referral to an agency that does PCA services.

What is required of me to keep receiving this service?

Alongside completing your annual Medical Assistance renewal and your annual waiver reassessment and annual, you will also be required to complete an annual meeting with our staff to review and update your paperwork.

Can my staff help me get to a fun outing?

Yes! Our staff can accompany you on occasional fun outings as long as they’re connected to your service goals and support your progress. 

If you’re looking for more frequent or non goal related outings, you can also ask us about our private pay services, which may offer more flexibility for social and recreational support.

How is this service different from other similar services?

Many services that are available on the waiver are services that are primarily focused on assisting by doing tasks FOR you. On the other hand, our service is more focused on meeting your goals ALONGSIDE you. Our services focus primarily on supporting you in learning and maintaining skills to help you remain as  independent as possible.

How do I know your staff is qualified?

All our staff undergo extensive background checks, on-the-job training, and ongoing coaching and check-ins. We only consider the most qualified individuals, so you can feel confident in the support you receive.
